=head2 signal_error

   $iterator->signal_error( $message );

Signal that an error occurred.  This version sets the
iterator's error flag and throws an exception.

The default handler throws an exception in the L<Iterator::Flex::Failure::Error> class.

The caller may define a custom error handler when the iterator is
created.  It must throw an exception.

If C<$message> is defined, it is passed to the error handler.
